Albums getting separated on iTunes

I have purchased complete albums that get separated or ripped into 2 separate playlist once I have them on my iTunes playlist. Some songs are even getting duplicated...this has been an issue for a long time and it's frustrating  because you can't listen to a whole album. Has anyone else experienced this issue or is this something that's happening under Apples radar that they might be unaware of? This happens on iTunes on Mac as well as on my iPhone.

iTunes sometimes displays a single Album as multiple Albums. Seemingly all fields are the same, but nothing that you do brings the Album under a single listing.
