iTunes has renamed about 80 albums to the same ...

Can anyone help me understand why about 80 albums in my iTunes have been renamed? It seems that the root folders have been changed – the moment I click to play an album it renames to "Loaded". This is happening every time I click to play a track, it just renames the album, so multiple albums are becoming one album with the wrong name. I can't figure out why it's happening and replacing the iTunes Library file doesn't solve the the problem as they've already been re-organised into folders?

I don't know what's happening, any help would be appreciated!
