Old Apple ID randomly appearing

I have the following products - iphone 6s, ipad mini, macbook pro. I use two factor authorization and recently had to reset my ipad back to factory settings so I'm reauthorizing all of my apps.

 

When I go to authorize imessages or itunes, the two factor authorization that pops up on my phone shows my old Apple ID. Everywhere I've checked on all of my devices (icloud, itunes, imessages, etc.) shows my current apple ID.

 

I went to manage my apple ID and typed my old ID in (the one that is appearing on two factor authentication) and it says it doesn't exist.

 

If I hit "authorize" on the screen that depicts my old ID, the authorization code appears and it works. Everything appears to be syncing, but why is the old ID appearing still. (I changed it about 2.5 years ago.)

 

It specifically says:

"Apple ID Sign In Requested [insert apple ID]. Your Apple ID is being used to sign in on the web near Philadlephia, PA.

 

Similarly, I keep getting a message that wants me to add the old email address to imessages, even though I keep deleting the email account. It says:

"Add [insert email address] for iMessage. The email address was recently added to your Apple ID [insert correct apple ID]." I just don't know why it's trying to add this email address.

 

Any ideas?
