Itunes and iPhone 6 not syncing apple music cor...

Just a quick one, I create a playlist on my phone and it pops up on the pc's iTunes, I create a playlist on the pc and it Doesn't show on the phone.

Same with adding music, If i add to a playlist on my phone it instantly shows up on the pc, if i add to a playlist on my pc, it doesn't show up on my phone.

What am i dong wrong?

I've logged in and out both devices, turned on and off home sharing.
