Unauthorized music purchase

I received an email about a Pandora subscription. I have never purchased one. In looking over my phone I found an unauthorized music purchase as well. How do I resolve these issues?

Where did the email come from.   Did it address you by your name or the name Apple know you by?

It appears Pandora subscriptions can only be made via their website so that would not be an Apple problem.
