Screen Sharing with CenturyLink

Been trying to set up Screen Sharing between my iMac and MacBook Pro.  I've set up Back to My Mac on both machines, and can share the screen from the iMac to the MBP from inside the network, but am unable to connect with the iMac when I'm outside my home network.

 

I've got the iMac set to never sleep and wake for network access.

 

When I try to connect I get an error message saying 'unable to connect - check that the computer is available and firewall settings'

 

I have CenturyLink as my ISP (only solution available), and was wondering if anyone out there has solved this for themselves.  I can access my modem settings remotely, but I don't know what needs to be changed.
