Macbook Pro 2013 folder with line through it

My daughters 2013 MB Pro is getting a folder with a line through it on start up after updating. This is the 2nd hard drive that this has happened to in the past month, and the 3rd time it's happened since she bought it new. The first time we just put a new 1 TB hard drive in it and it worked fine for a couple of years. When I pull the HD out I can plug it up to a USB and it still has all the files on it. What do I need to do to fix this problem. She is in college and needs this for school.
