Dock icons disappear (Sierra 10.12.6)

Sometimes when opening an app and right-clicking the dock icon and choosing „keep in dock“ the icons are replaced by a quotation mark after re-boot.

Second thing: When dragging the app from the app folder to the dock the icon will persist even after re-boot. Cool so far. But then when opening the app via dock a second icon appears for that app.

It only seems to happen with older apps. My Sierra is totally fresh installed, just got the new Mac few days ago. What can I do about that?
