No window for Apple pw and verification code wh...

When trying to sign into iCloud on my iMac, I enter my Apple pw, and then receive a verification code on my iPhone.  The message on my Mac then tells me to enter my pw and the code - but it does not provide a window for me to enter this information.  I've seen a previous question in which someone thought that two windows would appear, one for the Apple pw and the other for the verification code, but that's not my issue.  In my case there is no window at all.
