Help me with my Mid 2009 Macbook Pro. Can't aff...

One and a half years ago, my Macbook Pro Mid 2009 had finally taken a dump if you will. It was being so slow and the hard drive finally crashed and I lost everything because I did not back it up. I decided to “rebuild” my Mac to see if I could get it working again because that would be cheaper than spending $1200 on a new one. I have no tech experience and just ordered parts from iFixit and put in a new Crucial MX200 SSD 500 gb and 2 new Crucial DDR3 4 GB RAM. I also bought a new battery to try and get some better battery life. I watched youtube videos to reconfigure my computer and got it up and working great. It was fast again and running like a champ. So, this past weekend, my Mac all of a sudden got very slow and would not open anything. I then restarted it and the apple logo appeared and the loading bar got halfway and then my computer shut itself down. It did the same thing and wouldn’t even let me get to the command+option+R mode. So I scrambled and took my Crucial SSD out and was able to copy all of the contents I needed onto my Mac Mini. So I at least got all my files back this time. I decided to install the Crucial hard drive into my mac mini just to see what would happen. My Mac Mini did the same thing as my Macbook Pro did. It loaded halfway and shut itself down. So obviously my SSD has an issue. I was able to run disk utility on the mac mini with the ssd and decided to erase the entire drive. After that I installed it into my Macbook pro. I tried to Reinstall El Capitan. I got this message "An error occurred while preparing the installation. Try running this application again." So I ran like 3 more times and got the same thing. I then went to disk utility to run first aid and erase and I got the "couldn't unmount disk" message. I had the option to click the unmount button right away but nothing would happen and the button wouldn't respond. What do i need to do to get my SSD to work with my Macbook pro again??? Please help!
