Entries in webforms close Safari

I have been having ongoing problems with the Safari browser window closing when I am typing into webforms such as contact info, application forms and purchasing forms.

When I reopen Safari, it takes a minute but opens right back up to the page/form I was working on but repeated attempts to enter text into the form produces the same result.  I have experienced this problem on several websites over the past month.

I have a MacBook Pro (13 inch late 2011) running OS X El Capitan v 10.11.6, processor 2.4 GHz  intel Core i5
