Duplicates in iTunes and on the iphone

I have almost 1200 songs that are duplicated in iTunes. Some have the download cloud icon next to them. If I show in Finder I see the original name and the same name with a "1" at the end. Why is this happening? But in iTunes the song name is the same original name without the number at the end. I'd like to get rid of the numbered ones in iTunes in some easy manner.

 

So why are they duplicating so I can stop that moving forward?

 

How to easily remove them as described?
