Your MBP, even though you may have bought it in 2012, is a Late 2011 MBP.

I think you mean upgrade the 5400RPM HDD to a SSD. Yes. You need a 2.5" SATA SSD.

RAM can be upgraded to 16GB.

RAM and SSD are both recommended from Macsales/OWC or Crucial.
