my ipadmini  is locked,lost my  user ...

Dear ,

I have lost my ipad mini user name and password hence cannot open and it is locked , but I know my icloud user name and password as my ipad mini is locked please info me how to activate it,i have iPhone 4 active with me .kindly help me how to open my ipad mini which is locked,
