Spinning Wheel, complete lockups, HDD failure?

I've been having a pretty large issue with my mid-2011 iMac (27 inch, currently on Yosemite, updating to High Sierra soon). It's been having a lot of spinning wheels recently, all of a sudden. In any program (none in particular), it'll lock up with the spinning wheel for maybe 30 secs or more, becoming more and more common. Today was a rather large case, as I wanted to back up everything so I could possibly reinstall, when about half way through, one program locked up, then anything I clicked on would lock up, until everything including Spotlight, the time, the dock, Force Quit, and everything else but the mouse locked up, and showed no response to any key presses. So, I waited, and a few hours later it unfroze for just a little while, where I was able to restart. Upon restart, it was still semi-responsive, but I noticed every program (mainly Disk Utility, as I had unplugged my backup drive during the freeze to see if it was the issue, and that seemed to corrupt it) was running very slowly, with long wait times for stuff that normally takes seconds. After that I did a AHT test, and got the error 4MOT/40000003 HDD-____ or something similar, even with multiple re-tests. I tried resetting the SMC, which one forum post said to do, and still got the same error. At this point, I'm wondering, is this just a software issue that reinstalling will help, can I somehow fix it without opening it up or taking it into the Apple Store, or is this an actual HDD issue, meaning it needs replacement - and if it is, will apple replace it without charging us (It is most likely out of warranty)? Thanks!

I think it's also important to mention we can't lose anything on this computer, so we can't really reinstall at this point in time.
