iMac screen full of grey boxes and glitches

My 2009 iMac 27" has been great until this past week when it refused to go past a white screen upon boot-up. I booted off the original DVD and both Disc Utility and Hardware Test came up OK. So I decided to reinstall Snow Leopard, which is the OS the DVD came with. Erased the hard drive, installed Snow Leopard, and now have screen glitches galore. Big, grey boxes and other assorted oddities fill the screen. See this video of the start-up sequence.

Not sure whether I have a bad graphics/video card (wouldn't that have shown up in Disc Utility or Hardware Test?), or something else. Can't imagine it's software related, since I've erased the hard drive and reinstalled the OS.

If I take it to the local Apple Store, does anyone have an idea of what it will cost? Are there any tests I can run prior to doing that? And at what dollar amount is it just not worth it to repair this old machine?

Apple Stores do not charge for a diagnosis, however if your machine is older than a 2011 they likely will not work on it as it is considered vintage. However, you can take it to an Apple Authorized Service Provider in your area. The AASP in my area does charge for diagnostics but credits back the fee if they perform the work. BTW, if you decide a repair is not cost effective and you want to replace the computer consider getting an Apple Certified Refurbished Mac.
