Why are my Scanned Documents (Notes) not syncing?

Create a new scanned document on iPhone 6s+, the text will sync but not the scanned documents. Frequently, I get a blank container where the image should be and nothing else. Sometimes, if I scan multiple pages of a document, only the first page shows up. Anyone else seeing this?

I had the same occur to me over the weekend.  Scanned a document with the Notes app, went to my Mac to attach it as to an email and it was blank.
