Notes Folders not syncing to iCloud

This weekend, on my iPhone 6s, running iOS 10.3.3, I organized all my notes into folders. However, on my Mac Mini (running MacOS 10.12.6) and my iPad Pro 9.7" (running iOS 10.3.3) these folders are missing. All my note files are on my iPad and Mac, just not the folders. In fact, I can modify a note on my iPad and the changes show up immediately on another devices.  This proves sync is working at some level.  Why are the folders missing?  Thanks.

I just created a test folder on my MacBook and the test folder appeared on my iPad. So this must be an issue with my phone's syncing? IDK.

As it appears the problem is on the iPhone end, I would disable notes on the iPhone, then reenable it.

You'll likely lose all of the organization you did there.
