Has anyone ever not been able to erase there a ...

I turned off my computer I turned it back on and held down command+R and after the computer rebooted it took me to the reinstall page and I tried to reinstall the operating system however I was not able to. I went and looked at the Console and saw the Installer report and it looks like this:

You're probably trying to erase the wrong volume.
