Two factor Authentication...what happened?

Somehow with this turned on:

You can completely bypass it with one device. In this case my iPad Air 2.

My registered information had been changed.

My email been changed. 

My security questions had been changed.

Note I did not make any changes except update iOS to 10.x. That's when it all changed.
