Audio MIDI Setup kernel panic, every time.

Ever since updating from Yosemite to Sierra, I get a kernel panic when I do either of these:

1. Simply open Apple's included utility, Audio MIDI Setup.

2. With Adobe After Effects CC 2017 open, go to Preferences>Audio, and click the Settings button (I think that just tries to open Audio MIDI Setup).

It's reliable, repeatable, and instant. The moment anything related to Audio MIDI Setup appears, the mouse and keyboard lock, and within 1 second, kernel panic.

Troubleshooting steps I tried:

1. I tried logging in as a different user, a generic default user account that I keep for troubleshooting. The crash didn't happen when I opened Audio MIDI Setup, but instead showed the list of audio devices, and it allowed me to remove some aggregate audio devices. When I used the app's menu to choose to see MIDI devices, however, I got the same instant kernel panic.

2. Used Soundflower installer to uninstall it. Looked in /Library/Extensions to ensure it's gone. Rebooted. Same crash.

3. In Audio Hijack Pro, I removed the "Instant On" component, which AHP's dialog box showed as being incompatible. Reboot. Same result.

4. After steps 2 and 3, I repeated step 1. Same result.

The machine and OS: Mac Pro, 32 GB RAM, using latest mac os x Sierra 10.12.2 (16C67) on a machine that was recently updated from Yosemite. Other things seemed to work, so far, but I don't use MIDI much and haven't tried opening Ableton Live at all yet.

NO MIDI or Audio external hardware is connected.

I've isolated the problem to a single driver file, I believe: AppleMIDIUSBDriver.plugin in /System/Library/Extensions/.

The problem remains, however, because I don't have a remedy. I tried replacing this file with the previous version, from a Time Machine backup, but the same crash occurs.

My best workaround right now is to delete the file and hope that nothing I'm doing requires this function. Not good.

I have the same problem as well on my 27" iMac. I don't get the issue on my MacBook Pro

It is possible that there is a problem with that file. What is the timestamp, and version of it?

But, like I say in the first reply here, I tried an earlier version from my TM backup, and the crash happens when that driver's in place, too. Its time stamp is Sep  9  2014.

I don't have any others to try.

My Build is Sept 2016 and it does the same

September 2014 sounds like a version for Yosemite (or earlier).

I would try running the Combo Update for the current version of the OS (even if it is the same one you already have installed), and see if it replaces this old file with a newer version - though since someone reports the same problem with a new version, I guess the problem may lie elsewhere.

Can you run Etrecheck and post its report here? I'm guessing there may be a conflict with some other software.

I tried this with a clean install of the latest operating system and it does it.

By clean install, do you mean the Mac has nothing else installed on it but Apple's Mac OS X Sierra?

That's what I think you mean, but wanted to be sure, because I was wondering if non-Apple software might be involved, such as (for example) Audio Hijack Pro v2.x or even Ableton Live.

Yes totally clean install. I can replicate then with Virtual DJ Pro *latest version and with Adobe Lightroom as soon as a MIDI controller is selected.

Thanks for sharing that, James Stenlake. The fact that it happens on a machine that has nothing but OS X on it should turn out to be a useful clue.

I can replicate it also by open  applications/utilities/audio midi setup and then checking midi devices and the Mac instantly shuts down and restarts

Clean install means nothing but the OS.

As soon as you reimport your stuff you may have reimported whatever caused the issue in the first place.

Did you get to try opening Audio MIDI Setup *before* reimporting or reinstalling applications or settings?

If not, then I have to say that your clean install did nothing to solve or test the problem.

I suggest you just install the system on a separate drive, don't import anything, just create one new account, and open Audio MIDI Setup. I would be extremely surprised if *that* crashed.

I recommend you run Etrecheck and post its report here. I strongly suspect a software conflict with some third party software.

Yes this was a clean install and then opened midi setup. No other software was installed.

Can tell me how to run Etrecheck and I will gladly post the report here.
