mid 2012 Macbook Pro 13 startup issues

I have a mid 2012 MacBook Pro 13” 

Since I have owned it I would get or start up issues or it would take a long time to wake up from sleep, etc. i ignored it as it was manageable for the most part. In the last year it got so bad, freezing, occasionally not restarting, prohibited sign on start up, question mark file, the whole lot. I shelved and just used my desktop for a month. 

I need it now so I decided to back it up, wipe the hard drive and reinstall from the web. It’s worse than ever. It won’t even boot now (other than the first 2 times, post reinstall) I sometimes get to my user login but then I get the Apple and nothing. Sometimes I get the prohibited circle and slash symbol. Sometimes the question mark file symbol. 

I’ve gone into disk utility to “repair” the disk. Doesn’t help. I've done the Apple hardware test, says nothings wrong. I’ve done the software reinstall. Apart from booting up 2 times ok it has devolved back to where it was. 

If it has always had issues and is now worse regardless of software reinstall wouldn’t it be a hardware issue? Seems like the software can’t find the startup disk. 

I am fully prepared to purchase new memory, new hard drive, etc, just not sure that’s it if the hardware diagnostic said hardware was fine. 

To be clear I can occasionally (it doesn’t work every time I attempt) boot into recovery mode and to that hardware test mode, but cannot boot into the full operating system. Tried to boot into safe mode and wouldn’t go. Just froze.

Help please.
