You pay full price for a new phone. Unless you bought a third-party insurance policy, loss is not covered.

If it's not covered by a contract, the cost of a replacement unit, minus any money you get from your insurer(if any). Apple doesn't cover loss or theft.

If it is, check the contract or ask your wireless carrier.
