Why does preview show multiple copies of the same file in the thumbnails view?

There are no duplicates of this file on my laptop. The Table of Contents view only shows one copy, but the contact sheet view also shows multiple thumbnails of the file.

It doesn't matter whether I open just one file, or several files. If I close the file and reopen it, or close Preview and reopen it, the same thing happens.

This has been happening consistently for quite a while. I am running Sierra 10.12.6 on a mid-2011 Macbook Air now, but I believe the problem started in El Capitan. I did a clean install of Sierra a few months ago, I think it was 10.12.5, and haven't had any significant problems with it.

Any suggestions?
