iTunes ALWAYS duplicating songs on sync to iPhone.

Basically every single time I sync my iPhone a random assortment of songs will end up getting duplicated. If i change nothing and do a second sync these same songs will have three copies on the iPhone, and a few others may now have been doubled.

Making sure to completely wipe the iphone clean and sync still does not effect anything. Likewise uninstalling and reinstalling iTunes doesn't fix anything either.
