iPad stuck in headphone mode

My iPad I was stuck on headphones. I tried the following:

1) verified headphones working and built in speaker not working

2) volume control always indicated headphones even when they were disconnected

3) unplugged the headphones

4) verified 1 minute countdown timer sounded alert on built in speaker

5) verified volume control adjustment indicated headphones

6) in settings/general/accessibility/hearing set mono audio on

7) verified volume control adjustment indicated speaker volume

8) in settings/general/accessibility/hearing set mono audio off

9) verified volume control adjustment indicated speaker volume

speaker volume was restored.
