Duplicate Keychain listings in Keychain Access

Have an odd behavior. I have the Keychain Access Menu displayed in my menu bar. I have 3 different keychains. However, after every restart/login to the computer, one of those keychains gets duplicated in the listing. It is always the same keychain (called Token Protected). If I delete the com.apple.security.plist, on the next login, everything is back to normal. However, the next time I login after that, there will be duplicates of that keychain showing up in the menu. I have verified that the keychain is not actually getting duplicated within the ~/Library/Keychains folder.

I can go into com.apple.security.plist after each login and see where additional instances of that keychain have been added.

Any ideas on what is happening?
