How to fix my MacBook Pro with white startup sc...

I turn on my mac and the apple logo appears on a white screen. Then a loading bar appears for a few seconds, then goes to black. I have tried restarting, shift+R, shift+control+option, and nothing has worked. This isn't the first time this has happened. Last summer, I had the hard drive replaced due to a similar problem then 4 months prior I went in and had the hard drive replaced
