Apple ID lock engaged, but not by owner, whats ...

The Apple ID activation screen appeared on my iPad while I was browsing Fox News, saying that the owner activated a lock. I have had the iPad since it was new and have had it for over 5 years. I did not activate the lockout. Fearing that perhaps this was tied to the browser, I powered off the iPad and then back on, but the lockout screen reappeared. I plugged it into my iMac and iTunes showed the same screen. I next did a restore from my last backup (holding down the power button and home key to start the process), but the screen came back. It finally went away when I entered my apple ID and password into the screen, which I did not want to do in the first place, not knowing if the screen was real. I quickly used my computer to change my credentials and other information. Has anyone had a similar experience. Why would the Apple ID activation screen appear out of the blue?
