MBP keeps rebooting after apple logo screen

Hello,

I have a 13 inch MBP

2.5 Ghz Intel Core i5

2 x 4GB 1600MHZ DDR3 SDRAM

500GB Solid State (upgraded from 500GB Serial ATA HDD)

Yesterday when I turned on my MBP, it showed the apple logo screen on startup and then rather than seeing the booting progress bar go, it rebooted showing the apple logo screen again and again. The MBP just keeps rebooting and I have to eventually turn it off.

I booted the MBP in Safe Mode and I got the following (see attached picture).

Can anyone help me understand the error and what are the next steps to debug/resolve the problem?
