Push notifications stopped working on iPhone

I’ve used this configuration for years in order to send via another SMTP server, on my iPhone, iPad and Mac. Since upgrading to iOS 11, the push notifications are not working properly on the iPhone. The settings are the same on the iPhone, iPad, and Mac, and although the iPad and Mac work fine, I rarely get push notifications on the iPhone. Sometimes when I fiddle with the settings I’ll get notifications for a while but they soon stop again.  I'm at a loss for ideas how to fix it.
