Make a backup now, either Time Machine or or a clone. Your file system is corrupt. Boot into Recovery Mode and from the Utilities menu choose Disk Utility and repair your Mac HD volume. If it fails, try it a few times until it reports repaired or that it could not repair. Let us know.
