How to associate a device with apple ID in iTun...

I am literally sitting at my computer in tears of frustration.  All I ever wanted to do in this world is associate a device with an Apple ID in iTunes 12.6.2.  I can't find the "Manage Devices" function.  Heck, I can't find the magic "iTunes" dropdown in the magic "iTunes menu."

Can someone please send me a screen shot?  Apple Support pages are no help at all.

Do I have to download an app before I can do the association?
