How to add folders in "On my iPad"?

I have an iPad, iOS 11, and I can't find any way to create and manage local folders for file storage on the device.  For example, I have Outlook installed, and I have some email attachments I'd like to save to local storage for offline use.  If I try to do this, I can pick "On my iPad" as the target, but then there are no folders to save anything into, so it prevents me from saving anything.  I see nothing that suggests I can add a folder.

If I use the "Files" app, it doesn't even have "On my iPad" visible.  I found online references for how to fix this which suggested using the Photos app, then try to save any photo to any "On my iPad" folder which will then get "On my iPad" to show up in Files again, but I have the same issue there; Photos can't save an image unless there's already a folder to save it to, and I can't create a folder?!
