MBP Late 2015 to 4K 60Hz

I'm trying to connect a MBP Late 2015 to a Philips 34 BDM3490 UC. The monitor has HDMI 1.4, 2.0 and DisplayPort 1.2 with a resolution of 3440 x 1440. 

 

I have tried a HDMI to DisplayPort cable and no output on monitor. I have tried a Mini DisplayPort to DisplayPort and I get the resolution of 3440 x1440 but only at 30Hz. 

 

When checking the display settings, it's locked on 30Hz. I've also tried using quickres but same result. 

 

Any suggestions or feed back is appreciated.
